Incision technique

Inframammary incisions have the ability to cut down on overall time. The surgeon uses a stab-shaped 3 mm incision to insert an infiltration tube. Next, 150 mL of tumescent solution is injected through the incision to elevate the pocket and promote hemostasis.
Inclusion criteria
Inframammary fold is a crucial landmark in breast surgical. It marks the inferior breast border on the chest wall, and defines ptosis. The fold also provides inferior support for subpectoral implants, an important consideration to avoid implant migration. Additionally, the fold is important for aesthetic and reconstructive purposes.
Participation in this study is only possible for women who are at least 14 years old. Patients under 18 years of age must be accompanied with a legal representative, who will sign the FITC. Women who are pregnant or breastfeeding are also not eligible. Patients should be aware that participation in the study will not result in any financial incentives.
Risques
Inframammary procedures come with risks. Inframammary surgery can cause a double-bubble defect where the breast tissue meets the implant contour. This type of deformity can lead to a number of complications, including breast cancer.
Inframammary incisions are not as effective as those made at the periareolar or circumareolar crease. This is because sensory nerves in the lower pole of the breast are prone to stretching or cutting during inframammary incisions. Inframammary can cause more nipple neoplasm than periareolar and are therefore less aesthetic.
Disadvantages
The most widely used type in plastic surgery is the inframammary. This is due to its simplicity and visibility. It also allows direct access to submuscular, subpectoral, and subglandular planes. Inframammary is also a great option for placing symmetry and minimizing scarring.

Modified approach
The inframammary folding is an incision which extends from the chest wall down to the underlying tissue. The inframammary folding is closed with multiple layers of absorbable stitches that extend to the medial, and lateral edges. The skin disc is then moved toward the middle of the breast to restore the original nipple–areolar position. The wound is closed with smaller gauge sutures.
The incision will be made along the inframammary (which is far from the flap’s edge). Sometimes, special tools are used to harvest flap materials, such an endoscopic instrument or a retractor with a light system.
